John Raymond Kennedy, 47, is also accused of falsely using a Social Security number.
Kennedy was arrested Aug. 3 and fired the following day. He had worked with the NHP since 1989.
Court documents claim Kennedy fraudulently obtained a passport in July 1984 under the name of John Steven Staran, a 34-year-old man who had committed suicide four months earlier in Aurora, Ill.
U.S. Magistrate Robert Johnston has issued a summons for Kennedy, who is free on bond. He is scheduled to appear for an arraignment Aug. 21 before U.S. Magistrate Lawrence Leavitt.
