Nevada Check Up forms mailed

Families who return the forms by Sept. 25 will be able to enroll their children and get coverage beginning Oct. 1, said Chris Thompson, administrator of the Division of Health Care Financing and Policy.

Enrollment forms received after Sept. 25 will be processed for insurance coverage beginning on the first day of the following month, he said.

The medical coverage includes hospitalization, prescription drugs, vision, hearing, dental and other services.

The Check Up program is available to families without health insurance who have incomes up to 200 percent of the federal poverty level. For example, a family of four could have an income of $32,900 and be eligible.

Premiums for the program range from $10 to $50 per family per quarter, depending on income, regardless of the number of children covered.

"Parents should respond quickly to ensure that their children can have the health coverage they need as soon as possible," Thompson said.

An estimated 25,000 children in Nevada are eligible for the Check Up program, and applications are available throughout the state at more than 300 locations, including Family Resource Centers, New Baby Centers, libraries and many county and state social services agencies and health clinics.
